Abstract

완전도재 수복물은 뛰어난 심미성을 가지고 있어, 환자들의 심미적 욕구를 충족시킬 수 있다. 이러한 도재관을 제작함에 있어 CAD/CAM 시스템을 이용하는 것은 시간과 비용을 절약할 수 있는 장점을 가지고 있으며, 정밀도 또한 점차 높아지고 있다. 특히 변연적합도는 수복물의 성공 여부에 중요한 요소이고, 이 분야에 대한 실험적 연구는 많이 보고되고 있으나 임상적인 연구는 아직 미흡한 실정이다. 본 연구에서는 CAD/CAM 시스템의 하나인 Procera? AllCeram 시스템의 변연적합도를 실제 환자의 수복물 상에서 측정함으로써 임상에 사용되는 CAD/CAM 시스템의 변연적합도를 연구하고자 하였다. 단국대학교 치과병원 보철과에 내원한 56명, 101개의 지대치에 대해 코핑 상태와 최종수복물의 변연오차를 입체광학현미경으로 측정하고 비교분석하여 다음과 같은 결과를 얻었다. 실험 결과, 코핑의 변연오차 (53.84 ± 38.83 ㎛)보다 최종 수복물에서의 변연오차 (45.82 ± 30.84 ㎛)가 더 낮은 값을 나타났으며, 이는 통계적으로 유의한 차이가 있었다 (P < .05). 각 측정 지점간 비교에서 역시 모든 변연에서 코핑 의 변연오차가 최종 수복물에서의 변연오차보다 더 높은 값을 나타냈으며, 통계적으로 유의한 차이가 있었다 (P < .05). 구치부의 변연오차는 전치부의 변연오차보다 더 높은 값을 나타냈으나, 통계적으로 유의한 차이는 없었다. 설측 변연의 경우, 근심, 원심, 순측 변연의 오차보다 더 높은 값을 나타냈으며, 통계적으로 유의한 차이가 있었다 (P < .05). 이상의 결과로 보아 Procera? 시스템의 Allceram 관의 변연적합도는 임상적 허용 범위 내에 있었으며, 특히 도재 축성후 변연적합도는 더 우수해지는 것으로 나타났으며, 각 변연 중 설측 변연의 오차가 가장 큰 것은 주모형의 정보 입력시 순측 부위의 특성상 일어나는 오류로 사료된다. Procera? 시스템 Allceram 관 수복시 설측 변연에 더욱 주의를 기울이는 것이 좋은 변연적합을 얻는 데 도움이 될 것으로 사료되며, 앞으로 주모형 변연 부위의 변곡 정도에 따른 변연오차에 대한 연구가 필요할 것으로 사료된다.

Statement of problem: Use of all-ceramic prostheses fabricated with CAD/CAM systems is increasing in the dentistry. Marginal fidelity in produc-tion of all-ceramic restoration has important clinical implications and is a key consideration issue in CAD/CAM production as well.

Purpose: Theobjective of this study was to analyse marginal fidelities of Procera Allceram Crown.

Material and methods: On 56 patients treated with Procerasystem Allceram Crown at Dankook Dental Hospital, marginal discrepancies of 101 abutments were measured by stereomicroscope at coping and fi-nal restoration stages. Paired t-test and one-way analysis of variance on marginal discrepancy data were conducted to determine the presence of sig-nificant differences between measurement and measuring point stages.

Results: Marginal discrepancies of final restoration (45.82 ±30.84 ㎛) werelower than alumina coping (53.84 ± 38.83 ㎛). Furthermore, the differences were found to be statistically significant at 95% confidence level.Anterior marginal discrepancies were lower than posterior marginal discrepancies, but they were not statistically significant. Lingual marginal dis-crepancies were higher than other measurement sites, and the differences were found to be statistically significant at 95% confidence level.

Conclusion: Within the conditions of this study, marginal fidelities of Procera Allceram Crown were acceptable, and after porcelain build-up, mar-ginal fitness improved over alumina coping. More careful scanning is needed for better results.
